How do you treat palato-gingival groove?

Here is a case where #10 has consistent pus coming out on the distal side.  X-rays and probing show that perio pocket has reached the apex of the root.  The patient chose to have the tooth removed and replaced with an implant.  What options are there?

Notice the groove running down to the tip.

So the groove runs to the apex.  A highway for plaque.

Does CT attachment exist in the groove when healthy?

if the pocket was narrow enough to fix the bony defect, bone graft would be an option along with RCT. May be? :)

In my opinion, once a groove like this one becomes infected, it becomes very difficult to clean and thus makes grating very unpredictable.  I wonder if it is just my experience.  Anyone with better luck?

In my opinion, once the pocket gets deep, there is no turning back.  The tooth is doomed.
