Tooth #7 was fractured.  The root was removed and an implant was placed immediately.  The extraction of the root was performed by another dentist in the office.

Nobel Replace Tapered Groovy NP 3.5×13 mm was placed.  The patient appeared to have enough vertical bone to receive a 13 mm implant.  The extraction socket was drilled past the apical extent of the socket to ensure primary clinical stability of the implant.  The implant top was at about 3 mm apical to the CEJs of the adjacent teeth.

Temporary abutment from the same manufacturer was used.  The patient’s old PFM crown was used as the temporary crown, relined with acrylic resin.  Of course, the temporary crown was relieved of any occlusal contact.

It is regretful that the papillae were severed.

Please excuse the radiographs.  The temporary crown came loose once and was recemented.  The final crown was placed 6 months later.  The papillaes eventually filled in the embrasure space when the final crown was placed.

    So the temporary crown (the old PFM crown) will help mold the tissues around the crown immediately after the implant placement.
    It also saves a lot of time to use the old crwon.
